Add a Follow-up Visit Form

  1. Go to Data Management.
  2. Locate the patient data you wish to edit.
  3. Click on the arrow to expand the row and reveal additional options.
  4. Click on the Action button on the right.
  5. Select Add Form.
  6. Select the follow-up form you want to add.
  7. Click on the Add button. The follow-up form will be added to the patient's data.

After that, you can use the query feature to assign the form to a remote clinician. The remote clinician will then be able to access the form and complete it.
