Understanding Study Forms and Flow
The process begins with a Site User adding a new patient to the Talosix EDC system. Once added, the system automatically generates an enrollment form for the patient. The site user can then utilize the query feature to assign the patient to a remote clinician. Site Users can also add follow-up forms if needed.
Form Types
- The Registration Form is designed for entering essential patient information during the registration process. It collects details like the patient's name, contact information, and medical history.
- The Enrollment Form is automatically generated for each registered patient. This form allows you to upload patient documents. Additionally, you have the flexibility to customize it further using our form builder. You can add fields such as treatment details, clinical assessments, and any other data that is relevant to the specific study protocol. The form builder empowers you to tailor the Enrollment form according to the unique requirements of your study.
- The Other Form is a follow-up visit form that can be used to collect any data from patients at specific points after they have been enrolled in a clinical trial.
Form Status
Form status indicators provide a clear visual representation of the status of forms for a patient. You can hover over each indicator in the indicator legend to view their descriptions. There are three main statuses to look out for:
- Active: This status indicates that the form is currently active and ready for data entry. You can proceed to enter the required information in the form.
- Incomplete: When a form is marked as incomplete, it means that it has been started, and some field responses have been saved, but the form has not been submitted yet. Make sure to review and complete any remaining fields before submission.
- Complete: A form with the complete status indicates that all required data has been entered, and the form has been successfully submitted. No further action is needed for this form.
- Inactive: The form is currently unavailable for data entry. This status is assigned when the form falls outside of the designated data entry window.
